Without knowledge there are no benefits for patients

In an interview granted to EFEsalut, Dr. López Bran, head of the Dermatology Service at the Hospital Clínic Universitari San Carlos de Madrid, has emphasized that “the transmission of the knowledge and clinical experience accumulated by veteran doctors is fundamental to improving the overall health of the entire population” .

López Bran: “Without knowledge there is no benefit for patients”

“This education, focused first on undergraduate students at the Faculty of Medicine, then on internal resident doctors (MIR) and then on the youngest assistant doctors in the corresponding hospital specialties, must be systematic”, he adds.

What’s more, the theoretical and practical training of excellence, together with scientific and technological research, will always seek to meet and satisfy the individual needs of each patient”, completes the professor of Dermatology at the Complutense University.

“A magnificent example has been the first introductory course to hair transplantation organized in November 2022 within the 34th Congress held by the Spanish Dermatology-Oncology and Surgery Group (GEDOC) of the Spanish Association of Dermatology and Venereology (AEDV )”, reports.

The course, coordinated by López Bran himself and Dra. Alba Gómez Zubiaur, dermatologist specialist in Hair Medicine at the Ruber International Hospital, focused her main objective on “integrating hair transplantation into routine practice as a fundamental pillar of global medical-surgical care for the patient with alopecia”, he explains.

The entire GEDOC Congress, led by Dr. Francisco Javier Vázquez Doval, specialist in medical and surgical dermatology and reference in the surgical treatment of the skin cancer.

700 dermatologists debated the best treatments for melanocytic and other skin cancers (squamous and basal cell), as well as innovation in diagnostic and surgical strategies within the specialty.

We also analyzed cutting-edge biological, immunotherapy and targeted therapies that improve the prognosis of patients whose skin cancer is in advanced stages.

Likewise, in the surgery sessions, the content of twelve videos on new surgical techniques developed and evolved by dermatology specialists was studied.

Knowledge of a top hair transplant surgeon

“Those of us who have had the good fortune to join the teaching activity from a very young age have been transmitting our knowledge tenaciously to the university and constantly in our day-to-day hospital activity,” says the head of Dermatology.

“And if we talk about alopecia, the seed of medical interest is sown hair by hair in the younger assistant doctors of the Service, especially in those who want to know and develop this surgical technique, he indicates.

“In fact – he points out – the diagnosis that looks for the origin or possible causes of alopecia must be part of the patient’s medical and surgical treatment, taking a global and integral approach to their health problems.”

This therapeutic hole in health care is replaced on most occasions by attending courses, seminars or congresses at national or international level.

During the past GEDOC we also review current oncology. We know from different studies the direct relationship between early hair loss and the development of scalp cancers“, points out the dermatologist.

“There was no better academic environment for the pioneering course on hair transplantation. It allowed our assistants and MIR doctors to train in a technique that is constantly innovating, given that in the public health environment this type of technique is only used in very specific cases of cicatricial alopecia”.

Does the success of the course in hair transplantation reinforce the importance of alopecia in dermatology?

“Obviously. Trichological consultations are the most frequent reason for patients to visit dermatologists at certain times of the year”, he testifies.

“In addition, the number of young dermatologists who want to offer their patients a complete solution to their pathology is growing and growing, just as it happens in other sister specialties,” he emphasizes.

This has been the success of this first course in hair transplantation at GEDOC, also taught by Dr. López Bran, which will have its continuity in a workshop that will be addressed at the Clínica Imema in Madrid.

“The specialists, already trained in theory, will apply their practice in experimental models; they may even do some minor live surgical work. Voluntary patients will have the opportunity to correct their alopecia with modern transplantation techniques”, declares the microcapillary surgeon.

Ten essential knowledge from Dr. Eduardo López Bran for the success of a hair transplant

Quality theoretical training, the basis of the clinical development of the specialist in dermatology; although it will always suffer from the practical part at the university stage and in the hospital field.

Qualified practical trainingof the hand of a stay in a national or international clinical center of excellence. Tutored by experienced specialists and carried out practices in experimental models and live models, when their training allows it.

A good medical team. Surgical treatment of alopecia can only be done through the joint work of experienced health professionals.

A proper operating room in a safe environment. The physical assistance of the anesthetist is essential, preferably from the beginning to the end of the intervention for the safety and comfort of the patient, in addition to the tranquility of the entire medical and nursing team participant

Selection of the patient who obtains an accurate diagnosis and is an ideal candidate for a hair transplant. He is a patient who has requested a comprehensive approach to his androgenic alopecia or baldness.

Optimal design of the area to be treatedboth of the entire scalp on the head and of the donor and recipient parts of the hair follicles.

Safe and efficient extraction of follicular units. Always in a homogeneous and precise way, preserving the donor area (occipital area, especially the nape of the neck), since every day we encounter a greater number of young patients who request a first hair transplant at the first symptoms of baldness.

Androgenetic alopecia will progress as the patient ages. Transplantation allows repopulation of those areas where hair has been lost, but does not prevent the loss of surviving hair in the frontoparietal area of the head. There is a possibility that there will be a second transplant in the future.

Correct conservation of follicular units. Extracted hairy bulbs must “spoil yourself” during the time they remain outside the scalp and until they are transplanted to the recipient area or bald area. In addition, its activation will be appropriate.

Careful implantation of each hairwithout traumatizing the extracted follicular units and preserving their individual viability.

Follow-up and personalized post-operative treatmentalways at the hands of the specialist dermatologist, one of the keys to the best possible hair transplant.

With my years and years of clinical experience and the systematic training I receive and have received in my various academic stays in international centers, I try to transfer the most relevant knowledge to young people with a single goal: to obtain the best possible hair transplant for each patient.
